How to Submit a Client Application

Visit the carrier website (quick links above) to get copies of the appropriate application paperwork. Once the application is complete, scan and upload the application along with the signed illustration using the ‘Submit Application‘ page (see link on right sidebar). You will receive confirmation that your application has been received in 24 to 48 hours. Any incomplete or unsigned paperwork will be returned to you unprocessed.

Important Note

Before visiting SuranceBay (SureLC)

Please have the following items scanned to your computer so you can upload them:

  • Copy of your E&O insurance
  • Legal documentation and/or written explanation for any bankruptcy, tax liens, court judgments, or felony records. (This would be for any questions that you answer ‘yes’.)
  • Copy of your AML completion of your AML training.

If you do not have AML documentation available, have your LIMRA user name and password ready so you can access that information quickly. Please note that most carriers require AML training to be completed through LIMRA. If you need assistance with this or any other elements, please use the form below to contact Luci.
